Nine patients. Mean age 69. One three-day course of dasatinib and quercetin, two off-label cancer and supplement compounds taken together. Eleven days later the biopsies came back. Senescence-associated β-galactosidase-positive cells, one of the cleaner senescent-cell markers, were down 62 percent. The p16 cells were down 35. Circulating IL-6 and the matrix metalloproteinases fell too. That trial was published in 2019, it was open-label, it had no placebo arm, and it remains the most-cited human evidence that senolytics do in a person what they do in a mouse.

That is the floor the 2026 headlines were built on, and it is worth standing on it before reading them. Senolytics are drugs that clear senescent cells, the aged cells that stop dividing and sit in tissue leaking inflammatory signal. The 2026 results that observers called a clinical threshold are real, and they are small. Unity Biotechnology’s eye programme keeps posting a clean safety profile with an unresolved efficacy question. Mayo’s own cognition-and-mobility pilot this cycle ran twelve people, single-arm, no placebo, mean age 77. Gait speed barely moved. The Timed Up and Go improved by about a second, which the authors themselves flagged as not clinically meaningful. They called the study underpowered and hypothesis-generating, in those words.

Singapore is about to test that proposition with money. In May 2026 the government put S$350 million into a Grand Challenge on healthy longevity under RIE 2030, and Health Minister Ong Ye Kung framed it plainly: live long, live well. The gap he is buying against is roughly ten years, the distance between a Singaporean’s life expectancy near 83 and the point around 74 where good health tends to end. The demand is domestic, rich, and ageing faster than almost anywhere. One in five citizens is already past 65. By 2030 it is one in four.

Now look at what the wealthy 55-year-old actually buys. At Chi Longevity, the clinic geroscientist Andrea Maier co-founded inside Camden Medical and the Four Seasons in 2023, the diagnostics are real and the drugs are available. The load-bearing part of the model is quieter. The multidisciplinary team includes health coaches, and the method Maier built is a continuing relationship that reviews goals and adjusts the plan rather than a single prescription. This is the reframe the biotech headlines miss. The bottleneck in longevity was never the compound. Resistance training, Zone 2, sleep, and protein have Level 1 evidence a decade deep and cost nothing, and the wealthy 55-year-old does not do them consistently, because nobody does anything consistently without a person who notices when they stop. The senolytic is a two-day event. Compliance is the product. In a fast-ageing, high-trust, high-income city, the durable margin sits with whoever owns the relationship that gets a principal to show up in week 37.

China is running the same equation at a different scale. Its silver economy passed an estimated RMB 7 trillion by the end of 2024, and Beijing’s biotech policy names senolytics explicitly among the ageing interventions it wants to encourage. Policy is the operative word. The molecule scales with a factory. The habit scales with a person, and people do not IPO cleanly. That is precisely why the adherence layer stays underbuilt while the drug gets the funding round.

The uncomfortable read for a founder is that this is a boring business. It is a habit, a calendar reminder, and a person who notices when you go quiet. There is no defensible molecule at the centre of it, only a relationship that renews because it works and is hard to switch out of.

The senolytic will arrive, graded honestly, in its own time. The clinic that survives to prescribe it is the one that already knew you by then.
